The Social Security Act was originally signed into law in 1935.  Former President Harry S. Truman was the first member ever to enroll in the federal Medicare program.  Medicare is a federal health insurance program.  Federal statute specifies eligibility :

Because Medicaid is administered on the state level, eligibility varies depending on the state.  Medicaid is an assistance program designed to serve low-income individuals of every age.  Certain groups that are generally covered are pregnant woman and children living below the poverty level, individuals defined per statute as medically needy, certain people with disabilities.  The specifications for eligibility can vary so greatly it is critical to check on the state level.  

The Centers for Medicare and Medicaid Services (CMS) oversees Medicare, while private insurance companies actually administer the program.  Medicaid, on the other hand, is administered entirely on the state level.
